Over the past few decades, there has been increasing attention focused on the ethics of health research, particularly in low- and middle-income countries. Despite the increasing focus on the literature addressing human protection, community engagement, appropriate consent procedures and ways to mitigate concerns around exploitation, there has been little discussion about how the duration of the research engagement may affect the ethical design and implementation of studies. In other words, what are the unique ethical challenges when researchers engage with host (...) communities for longer periods (10 years or more), and what special considerations does this time commitment generate when applying ethical principles to these kinds of studies? This article begins to outline key areas of ethical concern that arise during long-term, sustained research activities with communities in low-resource settings. Through a review of the literature and consultations with experts in health systems, we identified the following key themes: fair benefits and long-term beneficence; community autonomy, consultation and consent; impacts on local health systems; economic impacts of research participation; ethical review processes; and institutional processes and oversight within research organizations. We hope that this preliminary exploration will stimulate further dialogue and help inform ethical guidance around long-term research engagements in the developing world. (shrink)

Dissecting criticism of Homer has proved to its own satisfaction that certain books of the Iliad are late, and have special affinity with the Odyssey. This Odyssean connexion is established by collecting verbal and metrical peculiarities and grammatical usages, which are found outside these parts of the Iliad only in what is held to be the later poem. The chief delinquents are I, K, Ψ and Ω but many would add the Apaté, Nestor's reminiscence in Λ, and other tracts. There (...) is not complete agreement as to the limits of the category; but generally it is assumed that the four books, and, when necessary, other parts of the Iliad, have the Odyssean taint. Human beings can be ethically frail under the pressure of situational forces, therefore the constraining force of the law is required. The ethics community need to have the confidence and courage to seek for the best ethical guidelines to become such constraining laws. However laws are themselves only ethical when they informed by a consensus that includes and represents the needs of the parties they are intended to protect, therefore the voice of the volunteer must be heard. Specific examples are (...) adduced to show how the current Clinical Trials Regulations are either insufficient in their scope, or not being fully met. To conclude eight suggestions are given as to how volunteers might be better protected. These include the establishment of an independent funded body to inform and support volunteers, a volunteers charter, greater consultation with volunteers, and a change in ethos involving a more egalitarian partnership between all those involved in the development of new medicines. A necessary condition for the development of a philosophical culture is the possession of a history of philosophy that conserves the experience of posing and discussing philosophical problems. Apologetics, dogmatism, a rigid devotion to the class approach, and ignoring universal human values for a long time dominated our social science and substantially deformed the way the history of philosophy was taught, giving rise to a number of stereotypes that hinder the revival of the skills of a culture of professional philosophizing.
